• Passport Photo: Affix a recent passport-size photograph of the pensioner in the designated box at the top left.
• Cross-Signature: The ASWO must sign across the affixed passport photo.
• Personal Details: Accurately fill in the applicant’s name, Village, Secretariat, and Mandal.
• Final Authorization: The bottom right of the form requires the full signature of the ASWO along with their official stamp.

Note: This certificate is strictly for pension sanctioning purposes and will be cancelled without intimation if any information is found to be false or ineligible.
